Infertility can be caused by one or more factors for both men and women. It is important to recognize the signs of when a couple should take the first steps to see a doctor to learn about their options. Most experts suggest at least one year of trying for men and women younger than age 35. However, women aged 35 years or older should see a health care provider after 6 months of trying unsuccessfully. For women, there are many reasons for infertility, many which involve some disorder that interrupts ovulation and the development and release of a viable egg. Some common reasons include: blockage of the fallopian tubes which can be caused by endometriosis, pelvic inflammatory disease, adhesions, scarring, a diminished ovarian reserve (fewer eggs available), ovarian failure caused by aging or other factors, a defect in the female anatomy that affects the movement of the egg into the uterus or prevents implantation.
Recently, research has found that environmental factors may play into infertility. It has been observed that a great increase in premature ovarian aging can be linked to different environmental factors. The tests that can determine if ovarian reserve is an issue are AMH (Anti-mullerian Hormone) and day 3 FSH measurements and ovarian ultrasounds.
For men, sperm problems may involve the quantity, quality, and/or motility of the sperm. Sometimes a man may not produce any sperm, or perhaps just too few sperm. In other cases, sperm may be malformed and die before they have a chance to reach the egg for possible fertilization. Abnormalities in the shape of the sperm are observed in the “morphology” part of a semen analysis test. There may also be problems with the delivery of sperm from the penis into the vagina may also be a cause for infertility. These problems include erectile dysfunction, problems with ejaculation, or blockage of the ejaculatory ducts.
